On-line Registration Form:

These forms must be filled out and submitted to Foothill Cremation & Burial Service. We will then mail them back to you for your signature. Payment should then be sent with the signed forms back to Foothill Cremation.
On each of the On-line forms, you will be required to provide your Name and Phone Number so we can keep all the forms together.
